Health Forms<\/strong><\/span><\/h4>\n

Prior to the start of the 2019-2020 school year, parents\/guardians are required to fill out annual health forms in Aspen.\u00a0<\/span><\/p>\n

Parents need to log into their parent Aspen account and click on Page Directory on left hand side.\u00a0 Click on Health Forms to fill out all required health forms for the new school year.\u00a0\u00a0All forms must be filled out in Aspen, with the exception of the physical form. The\u00a0original and signed\u00a0physical form must be dropped off or mailed to the health office (keep a copy for your records). The only exception is freshmen parents can turn in a photocopy of the physical as their child’s\u00a0previous school may have the original (freshmen must turn in a valid physical by August 1).\u00a0For a blank physical form, please visit the health office website – https:\/\/www.nhvweb.net\/vhs\/healthoffice\/<\/a>.\u00a0 Physicals are not mandated annually, unless the student participates in sports or intramurals.<\/span><\/p>\n

Chromebook and Network Forms for freshmen and transfer students<\/strong><\/span><\/h4>\n

The North Hunterdon-Voorhees Regional High School District has a 1:1 mobile computing device program through which every student is provided a Chromebook to use for schoolwork. Students are also provided with a school Google email address and Google Drive account. The district provides students with two options for Chromebooks: accept a Chromebook issued by the high school district, or bring your own personal Chromebook.\u00a0 Both options require an agreement form to be filled out in Aspen (more information on that below).<\/span><\/p>\n

 <\/p>\n

District-Issued Chromebook<\/u>:<\/span><\/p>\n

If students choose to accept a district-issued Chromebook, they will be provided the Chromebook, with case and charger, at Freshman Orientation on Wednesday, August 21, 2019. The Chromebook device has a battery that will last the whole day, an 11.6\u201d screen and 16GB local storage as well as unlimited storage \u201cin the cloud.\u201d\u00a0 The district\u2019s technology department centrally manages all the Chromebook devices.\u00a0 Devices do need to be turned in at the end of each school year for yearly maintenance and updates.<\/span><\/p>\n

 <\/p>\n

Bring Your Own Device<\/u>:<\/span><\/p>\n

Students can choose to bring their own Chromebook.\u00a0 The Chromebook would need to be submitted to the technology department at Freshman Orientation on August 21st<\/sup> so that it can be registered on the district\u2019s network; device would be returned to student within a couple of days. The district currently supports any Chromebook laptop device (to see options, visit https:\/\/www.google.com\/chromebook\/find-yours\/<\/a>, and filter by type – Laptops). Students cannot<\/u> use iPads, Android Tablets, MS Laptops, Surface, Apple laptops or Kindles devices. Students who use their own Chromebook do not have to submit the device over the summer for maintenance.<\/span><\/p>\n

Each high school has a Technology Resource Center to assist with any Chromebook issues throughout the school year.<\/span><\/p>\n

 <\/p>\n

Forms<\/u>:<\/span><\/p>\n

As mentioned above, parents\/guardians must fill out an agreement form for either Chromebook option<\/strong>.\u00a0 Form must be completed in Aspen (the district\u2019s student information system). Once you log into your parent Aspen account, click on Page Directory on left hand side, then click on Technology Forms. Please complete an agreement form prior to Freshman Orientation on August 21st<\/sup>. \u00a0<\/span><\/p>\n

 <\/p>\n

In addition to the Chromebook agreement form, all parents\/guardians must fill out the Acceptable Use Policy form<\/strong>, which enables your child to access the district\u2019s computer network.\u00a0 If you do not complete this form in Aspen, your child will be unable to connect to the internet or the school-issued Google account.\u00a0 This form is also located under Technology Forms in Aspen and must be completed by August 21st.<\/sup> \u00a0<\/span><\/p>\n

Lastly, under Technology Forms, please also review and complete the internet photo consent form<\/strong>.\u00a0 We would like to use student photos on our website, social media and with press releases as we publicize great news about our district.<\/span><\/p>\n
